[Indications and Effects]

1. In case of acute exacerbation of hyperammonemia due to the following diseases, or in case that control is not possible with oral administration, decrease in blood ammonia concentration

     1) Congenital factor circuit abnormality

- Carbamyl phosphate synthetase (CPS) deficiency

- Ornitine transcarbamylase (OTC) deficiency

- Arginosuccinate synthetase (ASS) deficiency

- Arginosuccinate lyase (ASL) deficiency

     2) Lysinuric protein intolerance

2. Acidifying agents in severe metabolic alkalosis

3. Pituitary function test to measure growth hormone secretion

- Use instead of or together with insulin-induced hypoglycemia test. This may appear as a false positive or false negative.

  Diagnosis is not made by drug testing alone.


[Uses and Dosage]

1. In case of acute exacerbation of hyperammonemia, blood ammonia concentration is reduced only when oral administration cannot control it

- 0.2~1g of L-arginine hydrochloride per kg of the patient's weight is slowly intravenously instilled over 1 hour.

2. Acidifying agents in severe metabolic alkalosis

- The dose for intravenous injection is L-arginine hydrochloride = amount of plasma carbonate concentration reduction to be reached (mM/L) × [patient's body weight (kg)/9.6]

3. Pituitary function test to measure growth hormone secretion

- Inject 500 mg of L-arginine hydrochloride per Kg of the subject's body weight as a 10% solution by intravenous drip over 30 minutes.

- Diagnosis method

1) Diagnosis in the morning after fasting and sleeping as usual

2) Lie down and rest for at least 30 minutes before injection.

3) After injection into the antecubital vein or other suitable vein, blood is collected from the vein of the opposite arm.

4) The blood sampling schedule is divided into 30 minutes, 0, 30, 60, 90, 120, and 150 minutes.

5) After the start of infusion, administer the recommended dose for 30 minutes without changing the dose.

6) Blood collected before radioimmunoassay is immediately centrifuged, and plasma is stored at -20℃.

7) The result of growth hormone deficiency is diagnosed by retesting this drug or insulin-induced hypoglycemia test. (The test interval is recommended after waiting for 1 day)
